Membership Disclaimer

Thank you for your interest in our Veterinary Clinic's membership program. By signing up for this membership, you acknowledge and agree to the following terms and conditions:

1. Veterinary Client-Patient Relationship: To be eligible for the membership, you must have an established Veterinary Client-Patient Relationship (VCPR) with our Doctor of Veterinary Medicine (DVM).
   
2. Payment Terms: Membership payments are billed monthly, starting on the date of sign-up, and continue for a total of 12 monthly payments (including the initial payment). Payments are automatically charged to the card on file. Memberships may also be paid in full at the time of sign-up, at the client’s discretion. If paying in full, no additional monthly payments will be required.
   
3. Cancellation and Service Adjustment: Should you choose to cancel your membership, or if your pet passes away during the membership term, services rendered up to that point (including exams) will be adjusted to the full non-member price. These charges will be applied to the card on file, up to the full cost of the membership. In the case that you have paid more than you have used, you will receive a refund for the difference based on the services rendered. Unpaid accounts will be sent to collections with a 24% interest rate.
   
4. Auto-Renewal and Life-Stage Membership Renewal: Monthly memberships will automatically renew on the one-year anniversary of your initial sign-up date unless cancelled. Upon renewal, your membership will automatically transition to the appropriate life-stage membership based on your pet's age or needs at that time. Cancellations require a minimum of one week's notice prior to the renewal date. For monthly memberships, this auto-renewal is continuous, but yearly memberships must be manually renewed each year by the member.
   
5. Exclusive Perks: Membership provides access to exclusive perks and bonuses not available to non-members. We are committed to serving clients who share our values and are dedicated to being active participants in our clinic’s community.
   
6. Membership is a Privilege: The membership program is a privilege and not a guarantee of continued membership. We reserve the right to remove members from the program if their values or participation do not align with the goals of our clinic.

Option 1: 10% off your pet’s COHAT procedure (includes cleaning and dental X-rays; excludes extractions)

Option 2: One (1) year of free Bravecto® (4 boxes, appropriate for your pet’s weight)

Eligibility

• Offer applies exclusively to COHAT procedures (comprehensive oral health assessment and treatment), which include cleaning and dental X-rays.
  
• Extractions, medications, fluids, and additional treatments are not included in the discount.
  
• To qualify, your pet’s COHAT must be booked and deposit paid between November 1 and December 31, 2025, regardless of when the procedure is performed.
  
• Bravecto® will be dispensed at the time of payment, once the COHAT has been completed.
  
• Required pre-surgical bloodwork must be done before dental procedure.
  

Bravecto® Terms

• Four (4) doses of Bravecto® will be provided, matched to your pet’s weight at the time of the procedure.
  
• Prescription parasite control is subject to veterinarian approval and discretion.
  
• Trenton Pet Hospital reserves the right to deny or refuse prescription parasite prevention based on medical judgment, health status, or patient history.
  
• If Bravecto® is not deemed appropriate for your pet, the 10% discount option will be applied instead.
  
• No cash value, exchanges, or substitutions permitted.
  

Additional Conditions

• Promotion cannot be combined with any other discounts, packages, or membership offers.
  
• Pets must have a current physical exam and be up to date on required vaccinations for anesthesia.
  
• Deposits are non-refundable if the procedure is cancelled without appropriate notice, as per clinic policy.
  
• Trenton Pet Hospital reserves the right to modify or withdraw this promotion at any time without prior notice.
